Healthy Communities

The goal of Healthy Communities is to promote:

• physical activity
• good nutrition
• tobacco cessation
• violence and unintentional injury prevention
• improve social isolation through policy and environmental change

Health Education

Health Education activities include:

• Community Health Assessment (CHA), every 3 years
• State of the County Health Report (SOTCH), annually except on CHA years 
• Community Health Improvement/Action Plans , every 3 years following CHA
• Community Outreach and Education
