Send Your Documents
You can use your myNCWIC portal (the most secure method), text, or email to send images of your required documents. These must be received before your appointment start time. A list of acceptable documents is provided below.

Submit Health Information
If you or your child have had a check-up within 60 days of your appointment, just let us know during the phone call. We'll be happy to fax a WIC Exchange of Information form to your provider for you!
Can't get to the doctor before your appointment? No problem! We can send you a form to take with you so your provider can send the information to us later. You can also stop by the Health Department, Monday - Thursday, 8 AM - 5PM, to have measurements quickly taken at any time.
